PA/FNP: Under the direction of the Chief Medical Officer, the Physician Assistant or Family Nurse Practitioner (both Board Certified), working within a patient centered care team, provides health care services to clinic patients utilizing professional skills in providing diagnosis and treatment of acute and chronic health problems along with preventative care focused on health risk factor reduction within the scope of licensing, training, and privileging/credentialing.  Provides care consistent with medical best practices and the policies/procedures/protocols of the Agency. Qualifications PA: Graduate of an accredited Physician Assistant program with a current and valid Arizona State license with prescribing authority; FNP: MSN and a graduate of an accredited Nurse Practitioner program with a current and valid Arizona State license with prescribing authority. Have a Valid DEA number, must be Board Certified. Ability to become credentialed with Canyonlands’ contracted health plans. 

Casual Part Time Customer Service Representative for Urgent Care– Provides Customer/Patient service to all patients of CHC facilities. Responsible for performing a variety of clerical duties relating to the day-to-day operations of the front office.  Greet patients in a professional and friendly manner.  Updating and organizing information in patient’s charts. Responding to inquiries and requests from staff personnel. High school diploma or GED required.  Moderate PC skills required with emphasis on accuracy. 

CNA – Full-Time – Urgent Care: Under the direction of the Practice Manager, the Certified Nursing Assistant (CNA) works with providers and other staff to provide quality primary health care services to CHC facilities. Completion of a Certified Nursing Assistant training program or Medical Assistant Certification. Must be CPR certified (BLS). Above average computer skills to operate and/or learn Agency’s current EMR/EHR system. Qualifications: One (1) plus years of directly related clinical experience.

Medical Assistant for Urgent Care- Three Full time at Urgent Care Works with physicians, midlevel providers and other staff to provide quality primary health care services to CHC facilities. Completion of a Medical Assistant training program accredited by the CAAHEP, ABHES, ACCSC, or other accrediting body recognized by the US Department of Education. CPR certified (BLS).

Full-time LCSW for LPMC. Responsible for the diagnosis, treatment, and maintenance of behavioral health patients.Licensed in the state of Arizona as a Clinical Social Worker.

Full-Time Registered Nurse for Urgent Care- Under the direction of the Practice Manager, the Registered Nurse (RN) acts as the lead clinical support staff member at his/her assigned site and is responsible for providing nursing care of a highly specialized and professional nature to clinic patients as well as providing support and assistance to medical providers, coordinating direct patient care while facilitating patient flow within the clinic. Acts within the scope of his/her nursing license provisions. Ability to perform each essential function duty satisfactorily with or without reasonable accommodation. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions, a Graduate of an accredited nursing program (ADN) required,  Valid, unrestricted, and current Arizona license as a Registered Nurse or compact as RN and CPR certified (BLS).
